Before the firm is able to fairly determine whether it may accept your case for evaluation at no cost, in your e-mail please describe your injury:  At a minimum, please provide:

1.    The date the treatment, conduct or act causing the injury occurred;
2.    Indicate in what city, state or province and country the injury occurred;
3.    If a doctor or other medical provider informed you that medical malpractice may have caused your injury, state the date this information was first provided.  If a doctor or medical provider did not inform you of possible medical malpractice, state how you became aware of possible medical malpractice in your case;
4.    Provide details about what the medical provider did or did not do that caused your injury;
5.    Describe your disability and any disfiguration suffered and indicate whether your disability and/or disfigurement is permanent

[for example: injury caused death, loss of limb, loss of hand or foot, permanant loss of use of the hand, leg arms, multiple finger or foot; permanent loss of sight to both eyes or hearing in both ears; life-time institutionalization in a psychiatric, medical or nursing home facility].

6.    State what you believe the amount of your wage loss is to date.
